Dear Secretary Barrios-Sanchez:
CBTS Technology Solutions LLC (the "Company") submits this Notice of Intent to Operate as an "Other
Telecommunications Service Provider"throughout the State of Idaho ("Notice")'. In support of this
Notice,the Company submits the following information:
1. The name of the telephone corporation and the business name of the telephone corporation if
it does business under an assumed business-name.
The Company's name is CBTS Technology Solutions LLC. The Company's Certificate of Existence
is attached as Exhibit A.
2. The United States and electronic mailing addresses of the principal place of business of the
telephone corporation, and, if there is a principal place of business in Idaho,the addresses of
