Additional Comments:
Observations included(2750 Soulen Drive, Emmett, ID): Safety equipment, JSA, valid dig ticket, verified
operator qualifications, verified equipment calibration, correct trench depth, observed butt fusion process,
verified proper rollback of completed 3/4"butt fusions, EFV 1200 installation, tracer wire,bedding with
native soil, successful pressure test, squeeze off, mechanical fittings (con-stab coupler),
backfill and continuity testing. Observed crews identify an AOC with a pre-installed leaking mechanical
fitting(con-stab coupler) the crew notified their District Operations Coordinator and supervisor and
replaced the mechanical fitting. All observed procedures were followed in accordance with IGC
procedures.
