Additional Comments:
Observations of IGC crew included (Hubbard/Linder Rd, Kuna, ID, Contractor struck unmarked
communication lines 2 times, immediately notified safety personnel, and then contacted CenturyLink to
arrange for repairs. Safety equipment, valid dig ticket, locating, verified operator qualifications, verified
equipment calibration, correct trench depth, observed butt fusion process, verified proper rollback of
completed 4" & 2"butt fusions, tracer wire and bedding with sand and backfilled with native soil. Quizzed
employee on applicable AOCs for rough handling times. All observed procedures were followed in
accordance with IGC procedures.
