February 22,2026: (3.0 Standard Travel) Galli, Green, and Brooks traveled to Salt Lake City, UT. February 23, 24, 25, 2026: (9.0 Standard) Galli, Brooks, and Green met with Enbridge employees Steve Bursett, Justin Berg, Lary Flemming, Weston Williams, and Josh Hunkele at the Enbridge Control Room along with Utah and Wyoming state programs. PHMSA's IA Question Set for Control Room Management was used as a guide throughout the audit to ensure all areas were completed in compliance with applicable regulations. Visited the control room for observations and conducted a controller interview. February 26, 2026: (3.0 Standard Travel) Galli, Brooks, and Green traveled back to Boise, ID. n'larch 2, 2026: (1.0 Standard Post) (Galli 0.5) (Green 0.5) Green and Galli wrote a compliance letter, completed paperwork, and reviewed IA question set. Discrepancies: Unable to provide documentation showing all aspects of point to point have been completed to include Hi Hi, Hi, Low, Low Low. Unable to furnish sufficient training of lessons learned of close calls (near misses), post-rupture investigations, operation and maintenance errors, emergency exercises or industry best practices will be integrated into training and procedures where applicable. Recommend changing language from should to shall regarding being away from the console for 15 minutes or more. Shift Handover Procedure 4.1, Recommend adding verbiage that states the primary and secondary server are not affected by development until the changes are pushed from the engineering server. IPUC Form 1 Rev. 12/08/2025 IPUC Pipeline Safety Inspection Recommend adding verbiage stating who has the final authority to initiate the switch between servers. Recommend adding verbiage stating the senior controller can be used as an advisor during on call hours and rest time will be adjusted accordingly.
