the Company is voluntarily relinquishing its Certificate of Public Convenience and Necessity to Provide
Local Exchange Telecommunications services in the state of Idaho originally approved on May 28, 2009 in
the above-mentioned case and order numbers.
The Company does not have any customers; therefore, customer notice is not necessary.
