Observations included (1325 18th Ave, Jerome, ID): Safety equipment, valid dig ticket, locating, verified
operator qualifications, verified equipment calibration, correct trench depth, observed butt fusion process,
stab fittings, electrofusion, verified proper rollback of completed 3/4"butt fusions, EFV 1200 installation,
tracer wire and bedding with native soil, successful pressure test, squeeze off, backfill and continuity
testing. Quizzed employee on applicable AOCs for fusion equipment inspections and rough handling
times. All observed procedures were followed in accordance with IGC procedures.
29-30 January. Observations included (1965 Blue Lakes Blvd, Twin Falls, ID,): Relocation of 2"meter set
and 2" service line approximately 3 feet. Safety equipment, valid dig ticket, locating, verified operator
qualifications, verified equipment calibration, correct trench depth, observed butt fusion process, met-fit,
and bedding with native soil, squeeze off. Quizzed employee on applicable AOCs for fusion equipment
inspections and rough handling times. All observed procedures were followed in accordance with IGC
procedures.
30 January. Observations included(1965 Blue Lakes Blvd, Twin Falls, ID) Safety equipment, verified
operator qualifications, pressure test, butt fusion, met-fit, bedding with sand and native soil, and backfill.
Traveled back to Boise and completed paperwork and forms. All observed procedures were followed in
accordance with IGC procedures.
