Observations included(3062, 3076 Elixir Rd, Ammon, ID): Safety equipment, valid dig ticket, verified
operator qualifications, verified equipment calibration, correct trench depth, observed butt fusion process,
EFV 775 installation, tracer wire and bedding with sand, successful pressure test and continuity testing.
Quizzed employees on applicable AOCs for squeeze off procedures.
Observations included(3043 Elixir Rd, Ammon, ID): Safety equipment, valid dig ticket, verified operator
qualifications, verified equipment calibration, correct trench depth, observed butt fusion process, EFV 775
installation, tracer wire and bedding with sand, successful pressure test, squeeze off, backfill and
continuity testing.
Discrepancies:
3076 & 3062: Inspectors found a damaged and dirty iron being used for fusion. Subsequent irons were
brought to the location, and both were found with damage or contamination on the heater faces. Damaged
pipe was found on the exposed service stub coming from the main line and left in service.
3043: Inspectors found improper rollback on 3/" fusions that were put into service and buried.
