Observations included (695 N Traquair Place, Meridian, ID): Safety equipment, JSA, valid dig ticket,
verified operator qualifications, verified equipment calibration, correct trench depth, verified proper
rollback of completed 3/4"butt fusions, EFV 1200 installation, tracer wire and bedding with sand,
successful pressure test, squeeze off, mechanical fittings (con-stab coupler), backfill and continuity testing.
Quizzed contract employee on applicable AOCs for squeeze off procedures.
Observations included(792 N Traquair Place, Meridian, ID): Safety equipment, JSA, valid dig ticket, hand
dig to locate service stub, verified operator qualifications, verified equipment calibration, correct trench
depth, verified proper rollback of completed 3/4" butt fusions, EFV 1200 installation, tracer wire and
bedding with sand, successful pressure test, squeeze off and backfill.
Discrepancies:
Identified expired PE piping being installed at 792 N Traquair Place. The expired pipe was dated 12-1-
2022 which directly conflicted with the IGC OPS 11 that states a maximum of 3 years. No UV
documentation was provided to prove the piping was protected from exposure. The expired PE pipe was
replaced with unexpired PE pipe to complete service line installation.
