October 6, 7, 20, 28, 29, 2025: (4.0 Standard Prep) (Green 1.5) (Galli 2.5) Inspector Green and Galli conducted a procedural audit of Intermountain Gas Company procedures. Procedures were reviewed and compared to the PHMSA Inspector Assistant(IA) question set. October 7, 2025: (0.5 Standard Prep) (Galli 0.5) Inspector Galli conducted an operator qualification audit. The PHMSA IA question set for the OQ portion was used as a guide throughout the audit to ensure all areas were completed as they pertain to required OQs. November 4, 2025: (1.5 Standard) (Galli 0.5) (Green 0.5) (Brooks 0.5) Inspectors Galli, Green and Brooks participated in a multi-state procedures audit of Intermountain Gas Company. _November 5,2025: (1.0 Standard) (Galli 0.5) (Brooks 0.5) Inspectors Galli and Brooks participated in a multi-state operator qualifications audit of Intermountain Gas Company, completed IPUC Form 1, and the Post-Inspection Written Preliminary Findings were sent to Colby Lundstrom, Manager of Operations Compliance. Discrepancies: 1.
