Galli and Perkins reviewed Montana Dakota Utilities (MDU) Control Room Management procedures
against the PHMSA Inspection Assistant(IA) and the 2022 IGC Control Room inspection results to
prepare for the 2025 Multi-State Control Room Audit with Montana, Wyoming, North Dakota, South
Dakota and Minnesota

Galli and Brooks travelled to Kennewick, WA.

Galli and Brooks met with MDU employees Colby Lundstrom, Wendy McDonough, Darren Tinnerstet,
Karmen Thompson, Molly Johnson, Mark Evans, Todd Robinson and Ron Kutzler at the MDU Control
Room along with other state programs. PHMSA's IA Question Set for Control Room Management was
used as a guide throughout the audit to ensure all areas were completed in compliance with applicable
regulations.

Galli and Brooks traveled back to Boise, ID.
Discrepancies:
Section 4.1 and 4.8 of OPS 631 omits any reference to the presence of a backup controller on duty during
weekday dayshift hours (Monday through Friday). IPUC inspectors recommend the following revisions:
• Acknowledge the presence of a backup controller during dayshift hours.
• Include the term "primary" when referring to the designated controller to clearly distinguish roles
and responsibilities.
• Strengthen the language to affirm that no individual may override or supersede the actions or
decisions of the primary controller.
In OPS 631, specifically when referencing API RP 1165, IPUC inspectors recommend adding the revision
number or incorporated by reference (IBR) per 192.7. The SCADA Screen Design Verification Form and
the Gas Control Loss of SCADA Communication Test Plan should also be referenced in OPS 631 where
applicable.
IPUC Inspectors recommend the removal of"significant" from section 9.1 so that control room actions are
considered when reviewing any incident that must be reported pursuant to 49 CFR part 191.
