Dear Mr. Brooks:
Here are the responses to the items of concern discovered during the inspection of Lower
Valley Energy's Crow Creek Pipeline project.
Items of Concern:
1. Procedures set forth in the Georg Fischer Electro Fusion Installation and training manual
Part number 10014339 Revision 9l2017 are not incorporated by reference and are not
included within the procedures referenced in this manual.
2. The installation was conducted without the use of the assembly clamps mandated by the
applicable manufacture's procedures and regulatory requirements.
Response:
LVE recognizes each item of concern. Each concern will be addressed below,
I. Welding, Fusing & Mechanical Fitting manual section H now revision 5. George Fischer
Installation and Training Manual has been referenced and included in the manual.
2. We have been using stab depth marks to make sure nothing has moved but we were not
using clamps. We ordered new clamps as we did not have any and will use them going
forward when fusing. All Gas Techs have been trained to use them going forward.
