Dear Secretary Barrios-Sanchez:
GW Operating, LLC (the "Company"), by undersigned counsel, submits this Notice of Intent to
Operate as an "Other Telecommunications Service Provider" throughout the State of Idaho. In
support of this Notice, the Company submits the following information:
1. The name of the telephone corporation and the business name of the
telephone corporation if it does business under an assumed business name.
The Company's name is GW Operating, LLC. It will conduct business in Idaho under the same
name. The Company's Certificate of Authority to Transact Business in the State of Idaho is
attached hereto as Exhibit A.

residential customers. All services will be provided pursuant to individual case-based contracts.
The Company intends to offer service throughout the State of Idaho, and accordingly, a map of
the Company's intended service area is not attached.
