Dear Secretary Barrios-Sanchez:
All West Communications (the "Company"), by undersigned counsel, submits this
Notice of Intent to Operate as an "Other Telecommunications Service Provider"
throughout the State of Idaho ("Notice"). In support of this Notice, the Company submits
the following information:
1. The name of the telephone corporation and the business name of the
telephone corporation if it does business under an assumed business-name.
The Company's name is All West Communications, Inc. The Company will
conduct business in Idaho under an assumed business name of All West
Fiber. The Company's Certificate of Existence from the Secretary of State of
the State of Idaho is attached hereto as Exhibit A.
