RE:Application to Voluntarily Surrender Certificate of Public Convenience and Necessity, CBTS
Technology Solutions LLC
Dear Ms. Barrios-Sanchez:
In accordance with Idaho Code § 62-612 and Idaho Admin. Code r. 31.01.01.020, CBTS Technology
Solutions LLC("CBTS") submits a request to cancel its Certificate of Public Convenience and Necessity
("CPCN") No. 509 to provide resold and facilities-based local telecommunications services in the State of
Idaho, issued by order 32677 on November 20, 2012, in Case No. BWT-T-12-01, as amended by Order
No. 33956 issued December 28, 2017.
CBTS does not provide services and does not have any customers in Idaho for the above-referenced
CPCN since January 1, 2025 and no longer requires its CPCN. Because CBTS does not serve any
customers pursuant to this CPCN, CBTS respectfully requests a waiver of the requirement in Idaho Code
§ 62-612(2) and Idaho Admin. Code r. 31.01.01.020 to "publish a notice of... withdrawal in a legal
newspaper circulated within the local exchange area." CBTS will continue to offer VOIP services within
Idaho.
If you have any questions, please reach out to me at Michael.Puza@cbts.com or by phone at 317-441-
9806.
Respectfully Yours,
CBTS TECHNOLOGY SOLUTIONS LLC
