Observation Summary—3232 SW 2nd, New Plymouth, ID
During excavation to locate the 1-1/4" gas main, Das-co personnel encountered a bare steel pipe.
Intermountain Gas Company(IGC)personnel were contacted to assess the line. Upon inspection, IGC
determined the pipe was a water line. They continued excavation and successfully located the 1-1/4" gas
main. While exposing the main, IGC personnel identified a leak on a single service meter(Meter
#426607), service tech was contacted to repair leak and determined that it was no longer in use the
isolation valves were shut off and locked to prepare the meter for abandonment. Das-co personnel
installed the steel line under the roadway and were in the process of finishing the welding to attach it to
the main. All observed procedures—including use of safety equipment, Job Safety Analysis (JSA),
verification of operator qualifications, equipment calibration, trench depth, sand bedding, welding, boring,
and line locating—were performed in accordance with the IGC Operations and Maintenance Manual.
