Observations included (5844 and 5864 W Oxton St (Split Service) Safety equipment, JSA, valid dig ticket,
verified operator qualifications, verified equipment calibration, correct trench depth, verified proper
rollback of completed 3/4"butt fusions, EFV 1200, bedding with sand, successful pressure test, squeeze
off, cleaning purge, backfill and continuity testing. All observed procedures were followed in accordance
with IGC Operations and Maintenance Manual.
Observations included (6229 N Foxglove Way, Eagle, ID): Line locating, safety equipment, JSA, valid dig
ticket, verified operator qualifications, verified equipment calibration, correct trench depth, observed butt
fusion process, verified proper rollback of completed 3/4"butt fusions, EFV 1200, bedding with sand,
successful pressure test, squeeze off, cleaning purge and backfill. All observed procedures were followed
in accordance with IGC Operations and Maintenance Manual.
