On behalf of Wavenation, LLC„ an affiliate of Airus, Inc., and Peerless Network of Idaho, LLC, please find
enclosed our petition seeking authority to operate as a Voice over Internet Protocol (VoIP) provider
within the State of Idaho. In connection with this petition and upon approval, we also respectfully
submit a request for numbering resources to support the provision of telecommunications services to
Idaho consumers.

NOTICE TO IDAHO PUBLIC UTILITIES COMMISSION
Wavenation, LLC (hereinafter"Applicant"), pursuant to Idaho Public Utilities
Commission("Commission") Rules for Telephone Corporations Subject to the Regulation of the
Idaho Public Utilities Commission Under the Telecommunications Act of 1988, 31.42.01 Rules
201-202, and Section 62-604(1)(b) of the Idaho Code, hereby submits this Notice to the
Commission for Authority to Operate as a V61P Provider within the State of Idaho. In support of
its Notice, the Applicant provides the following information:
a. The name of the telephone corporation and the business name of the telephone
corporation if it does business under an assumed business name;
The legal name of the Applicant and name under which the Applicant intends to do business is
Wavenation, LLC.Applicant is an affiliate of both Airus, Inc., and Peerless Network of Idaho,
LLC, both registered telephone corporations in Idaho.
b. The address of the principal place of business of the telephone corporation, and, if
there is a principal place of business in Idaho, the address of the principal place of
business in Idaho;

Applicant proposes to provide interconnected VoIP services across Idaho, enabling businesses
and residents to enjoy seamless voice communication over the internet while maintaining
connectivity to the traditional Public Switched Telephone Network(PSTN). Users can make and
receive calls to landlines, mobile phones, and other VoIP users, ensuring broad compatibility
across multiple communication platforms. While Applicant will be able to operate within each
LATA where it can obtain an LRN, it will offer coverage where underlying facility-based
providers serve.At present, these underlying providers serve all LATAs within Idaho, including
the Boise and Pocatello LATA(652), and the Coeur d'Alene LATA(960). Please refer to
Attachment for a map of proposed service areas.

Wavenation, LLC ("Wavenation"), an affiliate of Airus, Inc. and Peerless Network of Idaho, LLC, is
requesting authority to request numbering resources as an interconnected VoIP provider in Idaho.
Pursuant to the Federal Communications Commission's FCC Order 15-70, allowing interconnected VoIP
providers to obtain resources directly from the North American Numbering Plan Administrator and/or
the Pooling Administrator, Wavenation hereby notifies this Commission of its intent to request the
resources identified below. We are providing this notice at least 30 days prior to requesting numbers
from the Numbering Administrators.'

Application identified herein has been found, upon initial review,to be acceptable for filing as a
streamlined application." Moreover, we are providing a copy of FCC Order DA 21-282 in WC Docket No.
20-402, "Notice of Interconnected VoIP Numbering Authorization Granted," released on March 8, 2021,
wherein the Wireline Competition Bureau granted Wavenation, LLC interconnected VoIP numbering
authorization (see attachment B).

WaveNation,LLC (WaveNation)is an interconnected Voice of Internet Protocol(VoIP)provider
and filed an application pursuant to section 52.15(g)(3)of the Commission's rules, seeking authorization
to obtain North American Numbering Plan telephone numbers directly from the Numbering
Administrators.2 In its Application,WaveNation indicates that it intends to initially request numbers in
New York, Illinois,Florida, and Texas.3
In its Application,WaveNation, includes the contact information and acknowledgements required
by section 52.15(g)(3)(i)of the Commission's rules.4 WaveNation provides evidence that it will be
capable of providing service within 60 days of the numbering resources activation date.5 WaveNation
also certifies that it complies with the contribution,regulatory fee, and 911 obligations set forth in section
52.15(g)(3)(i)(E).6 In addition,WaveNation certifies that it has the financial,managerial,and technical
expertise to provide reliable service.' WaveNation further certifies that none of its key management and

WaveNation certifies that no party to the application is subject to a denial of Federal benefits pursuant to
section 5301 of the Anti-Drug Abuse Act of 1988.9
GENERAL INFORMATION
The Numbering Authorization Application identified herein has been found,upon initial review,
to be acceptable for filing as a streamlined application. The Commission reserves the right to return any
interconnected VoIP Numbering Authorization Application if,upon further examination, it is determined
to be defective and not in conformance with the Commission's rules and policies. Pursuant to section
52.15(g)(3)(ii)of the Commission's rules, interested parties may file comments in WC Docket No. 20-
402 on or before February 20,2021.10 Commenters must serve a copy of comments on WaveNation no
later than the above comment filing date. Unless otherwise notified by the Commission,WaveNation
may provide the states in which it intends to request numbers the required 30-day notice on the 31st day
after the date of this notice.I'
