(1) Consider adding stairs or similar up the embankment towards ESD buttons. -New ESD was installed closer to tank in case of emergency. 2. (4) Verify if on-board generator will shutdown when ESD is activated. -ESD Control switch on vaporization trailer was in the field vs plant position, once it was switched to plant position all ESD's will now shutdown plant and generator, IGC is installing a placard by the switch to tell personnel to leave in plant position. 3. (5) ESD station 817 between truck unloading area and control room, has been damaged, including a broken grounding cable. -New ESD switch, pole on which it is mounted has been replaced and grounding wire has been reattached. 4. (7) Control room has fuel gas for building heat but no gas detection. -Gas detection sensor and alarm has been installed. 5. (8) Shop area of the control building is unfinished (wood construction with exposed insulation). -All walls have been covered with sheet rock. 6. (9) Propane cylinder and gasoline can are stored on the mezzanine floor with could facilitate a fire. -All combustibles are now stored in a flammable locker on the shop concrete floor. 7. (10) Consider adding second gas detector closer to building heater to detect leak of fuel gas. -Gas detection sensor and alarm has been installed. 8. (12) Only CO2 fire extinguishers are available in the shop area, which may not be effective for a Class A fire. -There are now 3 Class ABC extinguishers in the building. Perimeter fence has significant gaps underneath in some locations and no security beyond fence (no motion detection, no security cameras). -Interior fence line has had gravel added and exterior fence line has had larger rock added to eliminate gaps under fence. 10. (18) Install no smoking sign specifically at unloading station. -New sign has been installed. 11. (19) Current extinguishers are not high flow. -Wheel mounted fire extinguishers are high flow. 12. Conducted an inspection on all concrete/structural issues previously identified. Structural condition assessment for the concrete cracking at the piers for the LNG tank and the containment structure walls as identified by IPUC during 18 October 2023 field audit and Forsgren Associates Inc. inspection report on 28 June 2024. (PHMSA Enforcement Item) -IGC contracted ESI to make the needed repairs to all concrete structural issues. Currently the contractor has started sealing the cracks on tank supports and piping supports. All cracks in piping containment and main spill containment are being sealed or repoured. Once repairs are completed the contractor will coat all concrete with a layer of water repellent/sealer. Will conduct follow-up inspection in the future when all concrete work is completed. 13. (6,14) items have not been completed and will reinspect in the future. June 24, 2025: Inspector Perkins completed all forms and loaded pictures.
