HomeMy WebLinkAboutLower Valley Energy 250626 LVE Project Meeting Construction IPUC Pipeline Safety Inspection Inspector(s): Perkins Date(s): 250625 Company/Owner: Lower Valley Energy (LVE) Unit/OPID: 31044 Standard: ® Training: ❑ TIMP/DIMP: ❑ OQ: ❑ Incident: ❑ Damage Prevention: ❑ Drug & Alcohol: ❑ Inspection Days: 2.0 Inspection Summary June 25, 2025: (1.0 Standard) Inspector Perkins traveled to Afton, Wyoming to meet with Craig Coles of Lower Valley Energy and Perry McCollom from the Wyoming Public Utilities Commission to discuss the Crow Creek Road project, which spans from Afton, Wyoming to Montpelier, Idaho. During the meeting, the project scope was reviewed in its entirety, including a detailed examination of route maps and proposed infrastructure. Inspector Perkins requested the Operator Qualifications (OQs) for all personnel involved in the Idaho installation, the Operator Identification Number(OPID),pipe specifications, locate tickets, comprehensive project checklists, and various other key deliverables related to the execution of the project. Following the meeting, we conducted a site visit in Afton where Lower Valley Energy(LVE)had begun pipeline installation. Earth Energy was actively engaged in boring operations under roadways. During the inspection, I reviewed the pipe being utilized for the project(refer to attached photographs) and identified damage exceeding 10%, likely incurred during offloading from the pipe trailer. I discussed the issue with LVE personnel, emphasizing the necessity of removing compromised sections and implementing preventive measures to mitigate future damage. WL Plastics (WL 101) Joining& Field Procedures For Pipe, Pipe Unloading and Handling procedure states to "Use only appropriate lifting and handling equipment that is in safe operating condition, that is properly rated for the load, and that is designed for the intended use. Do not lift or move WL Plastics products with equipment such as wire rope or chain that can damage the pipe. Equipment that contacts the pipe should be fabric slings or should be rounded or padded to prevent damage to the pipe." Additionally, we inspected the McElroy fusion equipment intended for joining pipe sections. LVE personnel noted that this was the third unit used on site, indicating previous challenges with the machinery's performance. We then proceeded to a location on Crow Creek Road where a contractor had installed approximately 200 feet of pipe, which had already been backfilled. The need for adequate pipeline markers along the route was also addressed during this segment of the visit. To conclude, I drove the proposed route of the pipeline—an estimated 20 miles along Crow Creek Road passing through the Caribou National Forest to Montpelier, Idaho. However, approximately three miles of Forest Road 801 (a bypass of Crow Creek Rd) proved impassable due to poor road conditions, preventing a complete survey of that stretch.
