COMES NOW Ziply Fiber of Idaho, LLC, and Ziply Fiber Northwest, LLC, both d/b/a
Ziply Fiber, by and through its counsel of record, C. Tom Arkoosh and Nicholas J. Erekson of
Arkoosh Law Offices, and hereby submits this Notice of Discontinuance of Service pursuant to
Idaho Code § 62-612 and states as follows:
1. Ziply Fiber is a telephone corporation operating and providing basic local exchange
and message telecommunication services within the State of Idaho, including in the Elk City
exchange area. Following an internal review,Ziply Fiber has determined that only three customers
remain within a large portion of this exchange area and that each of those customers currently
receives equivalent voice and broadband service from an alternative provider. The specific
townships and ranges affected are identified in Paragraph 2,below.
2. Pursuant to Idaho Code § 62-612(1), Ziply Fiber hereby provides formal notice of
its intent to discontinue the provision of regulated telecommunications services in the following
areas within the Elk City exchange area:

3. Alternative corporations are presently furnishing the respective or equivalent
service in the affected area. Specifically,ViaSat Carrier Services, Inc., offers voice and broadband
connectivity to customers via satellite technology and is capable of providing equivalent
telecommunications service to this local exchange area.
4. Consistent with Idaho Code § 62-612(2), Ziply Fiber will publish this notice in a
legal newspaper of general circulation within the affected local exchange area.
5. Pursuant to Idaho Code § 62-612(3), any person or telephone corporation may,
within 30 days from the date of publication of this notice, file an application with the Commission
requesting a determination as to whether the discontinuance of service should be authorized.
6. Ziply Fiber remains committed to minimizing disruption. As the only affected
customers are currently served by other service providers and Viasat Carrier Services, Inc., is
actively providing equivalent service,this notice is filed in compliance with Idaho Code § 62-612.
7. WHEREFORE, Ziply Fiber respectfully submits this notice in accordance with
