I, JESSICA EPLEY declare and state as follows:
1. I am the Vice President of Regulatory&External Affairs for Ziply Fiber Pacific,LLC d/b/a
Ziply Fiber("Company").
2. 1 make this declaration based upon my personal knowledge and review of records
maintained in the ordinary course of business by Company and Company affiliates
("Affiliates") that, like Company, are owned, directly or indirectly, by Northwest Fiber,
LLC d/b/a Ziply Fiber.
3. Company will offer and provide Lifeline services in the State of Idaho using facilities
owned by Company and/or facilities owned by Affiliates or a combination of those
facilities and resale of Affiliates' services.
4. The facilities owned by Company and/or its Affiliates in Idaho include central office
buildings, structures, and related network equipment that form part of the network which
Company will use to offer and provide Lifeline services.
5. The locations of certain facilities that will be used to offer and provide Lifeline services in
Idaho are identified on the map attached hereto as Confidential Exhibit A. This exhibit
depicts facilities that are 100% owned by Company and/or its Affiliates.

6. Attached hereto as Confidential Exhibit B is a true and correct copy of an Idaho state tax
document created in the ordinary course of business by Northwest Fiber LLC d/b/a Ziply
Fiber for the 2024 tax year in connection with tax compliance. Northwest Fiber, LLC, is
the parent company of Ziply Wireless, LLC, and the indirect parent of Ziply Fiber Pacific,
LLC. The"Tangible Personal Property Exemption" section of that document lists tangible
property in Idaho owned by Company and/or its Affiliates.Company will offer and provide
Lifeline services in Idaho using some or all of that property.
7. Attached hereto as Confidential Exhibit C is a true and correct copy of a document
entitled "Ziply Fiber Mileage Real and Other Property Not Apportioned," created in the
ordinary course of business in connection with property tax compliance,which lists certain
buildings, tower structures, and other property in Idaho owned by Company and/or its
Affiliates, which will be among the facilities used to offer and provide Lifeline service.
8. Attached hereto as Confidential Exhibit D is a true and correct copy of a document listing
owned and leased property addresses,which was created in the ordinary course of business
in connection with property tax compliance, that lists the addresses of structures in Idaho
that are owned (or, if indicated in the document, exclusively leased) by Company and/or
its Affiliates in Idaho,which will be among the facilities used to offer and provide Lifeline
service.
9. Attached hereto as Confidential Exhibit E is a true and correct copy of Company's Idaho
Emergency Preparedness, Backup Power, Disaster Recovery and Business Continuity
Plan.
