September 16, 2025: (1.0)
Traveled from Idaho Falls to the site located at 3868 E 400 N, Rigby, Idaho, following identification of a
potential coating anomaly through a Direct Current Voltage Gradient (DCVG) inspection. Upon
excavation, it was determined that a welded tapping tee exhibited improper wrap application, indicating a
possible holiday in the protective coating. To assess the integrity of the affected area, IGC engaged Acuren
to perform non-destructive testing (NDT) and a comprehensive integrity inspection over a segment
approximately 3 to 4 feet in length. Following completion of the inspection, KV Utilities rewrapped all
exposed sections, ensuring proper bedding and subsequently backfrlled the trench to restore the site.

Traveled from Idaho Falls to the site located at 2286-2298 W 1000 S, Rexburg, Idaho, following
identification of a potential coating anomaly through a Direct Current Voltage Gradient (DCVG)

possible holiday in the protective coating. To assess the integrity of the affected area, IGC engaged Acuren
to perform non-destructive testing(NDT) and a comprehensive integrity inspection over a segment
approximately 3 to 4 feet in length. Following completion of the inspection, KV Utilities rewrapped all
exposed sections, ensuring proper bedding and subsequently backfrlled the trench to restore the site.

file.
