A comprehensive field inspection was conducted at the above location, with the following key elements
verified and documented:
• Proper use of safety equipment and adherence to Job Safety Analysis (JSA)protocols
• Confirmation of operator qualifications and equipment calibration
• Verification of trench depth compliance
• Inspection of completed 3/"butt fusions, including proper rollback procedures
• Installation of tracer wire and appropriate bedding materials
• Successful completion of pressure testing
• Execution of squeeze-off and backfill operations
• Employee knowledge assessed regarding squeeze/release timing procedures
