IPUC inspectors Galli and Brooks conducted an in-person Control Room audit of the Avista Utilities
Control Room in Spokane, WA.
August 27, 2025, September 2,3,4 2025: (4.0 Standard) (4.0 Galli)
Galli reviewed Avista's Control Room procedures and records. PHMSA's IA Question Set for Control
Room Management was used as a guide throughout the audit to ensure all areas were completed in
compliance with applicable regulations.
September 10, 2025: (1.5 Standard) (1.0 Galli) (0.5 Brooks)
Galli and Brooks met virtually with Avista Utilities employees Randy Bareither, Nicola Hostetler, Brian
Zemke, Carie Mourin and Josh Fort along with Oregon and Washington State Programs to go over any
discrepancies. Completed audit forms: PHMSA IA question set, IPUC Form 1, Post-Inspection Preliminary
Written Findings and attached inspection documents.
Discrepancies:
During the observation there were two controllers at the desk. However, there is no language in the CRM
plan stating which controller is primary and secondary and the distinction of duties.
