July 28,29,30 2025: (2.5 Standard Prep) (Galli 2.5)
Inspector Galli conducted a distribution records audit of Intermountain Gas Company(IGC). IGC OPS
procedures and records were reviewed and compared to the PHMSA Inspector Assistant(IA) question set.
August 14,2025: (1.0 Standard) (Galli 1.0)
IPUC Inspector Galli of the Idaho Public Utilities Commission met with IGC compliance and engineering
personnel virtually to review records for the previous year of 2023 & 2024. A complete list of attendees
can be found in the IA inspection. Completed IPUC Form 1 and the Post-Inspection Written Preliminary
Findings were sent to Colby Lundstrom, Manager of Operations Compliance.
Discrepancies:
Report 450-26473 shows a 5-minute pressure test from 10:15 to 10:20 and it should have been 15 minutes
minimum.
Multiple Forrn 512's that did not specify minimum distances from underground utilities in them. Reports#
117263-425, 11711-375, 51101-371, 09372-367, 20607-613, 04785-575, 21494-583 had no indication of
proper underground clearances.
No purging was annotated while reviewing samples of abandoned mains and SL (SL-512, Main-319)#741-
3288, 741-3305, 741-3331, 679-2539. The check boxes were not checked indicating that purge requirements
were not met.
