Inspection Summary
Audit consisted of IPUC inspector Galli inspecting field installed equipment for the Idaho Falls, Idaho
inspection unit. The PHMSA IA question set and IGC equipment listing were used as a guide throughout
the audit to ensure all areas were completed/covered.
August 4,2025: (1.0 Standard MG)
Traveled to Idaho Falls and met with IGC company representatives in the Idaho Falls District Office for an
in brief and to begin field audit and review OQ records for all personnel taking part in the inspection.
Aup,ust 5-6, 2025: (1.5 Standard MG)
Regulator Stations: 20 regulator stations were inspected for proper operation, regulator lock-up, relief
device operation, cathodic protection pipe-to-soil readings, pipe-to soil interface wrap protection, station
warning signs and right-of-way clearances.
20 casing-to-carver isolation checks, 8 cathodic protection rectifiers, 9 pipeline patrols, 22
emergency/isolation valves and 35 HPSSs were checked for proper operation, observations listed below.
An exit interview was performed following the district inspection with field personnel of Intermountain
Gas Company and MDU Compliance Manager Colby Lundstrom.
Discrepancies:
48747-RS-60805 —Primary regulator did not lock up on the first test. Debris found on pilot stem and
diaphragm.
48747-RS-60770 - Primary regulator did not lock up on the first test. Debris found on pilot stem and
diaphragm.
48741-RS-59810—Secondary regulator did not lockup on the first test. Debris found on pilot stem.

Concerns:
Valve 48747-VLV-29560 was designated as a two-man valve in Maximo. However, technicians were only
able to move the valve key a few inches with significant force. This raised a concern regarding the valve's
operability, particularly in emergency situations.
48747-HPSS-67800 was found with possible pitting on the low-pressure side of the HPSS.
August 7, 2025: (1.0 Standard MG)
Travelled back to Boise. Completed and attached audit forms: PHMSA IA question set with OQ protocol
9, IPUC Form 1, Post-Inspection Preliminary Written Findings and attached inspection documents.
