AFFIDAVIT OF QWEST CORPORATION D/B/A CENTURYLINK QC (SOUTHERN IDAHO)
(COMPANY)
The Idaho Public Utilities Commission Order No. 29841 requires that Eligible Telecommunications Carriers
(ETCs) certify that they are compliant with applicable service quality standards and consumer protection rules;
and ETCs must demonstrate the ability to remain functional in emergencies. In addition, the Commission must
file an annual certification with the Universal Service Administrative Company and the Federal Communications
Commission (FCC)that all federal high-cost support provided to ETCs within the State of Idaho will be used
only for the provision, maintenance, and upgrading of facilities and services for which the support is intended.
Accordingly, the undersigned states and verifies under oath the following:
1. I am Senior Manager Regulatory Finance for Lumen Technologies Service Group, LLC, which is a
subsidiary of Lumen Technologies, Inc. ("Parent"). Under Parent's policies and a delegation of signing
authority by an officer of the Company, I am authorized to execute this affidavit on behalf of the
Company. The Company is an eligible telecommunications carrier and will be receiving federal
universal service support under section 214(e) of the Telecommunications Act of 1996 in the state of
Idaho.
2. 1 am familiar with the Company's day-to-day operations in the state of Idaho and with the State's
service quality standards and consumer protection rules as set forth in Commission Order No. 29841.
3. The Company is substantially complying with applicable service quality standards and consumer
protection rules of the Federal Communications Commission and the Idaho Public Utilities
Commission.
4. 1 certify to the Commission that the Company is able to remain functional in emergencies as set forth in
Commission Order No. 29841 and in 47 C.F.R. § 54.202(a)(2).
5. 1 also certify that all federal universal service support funds received by the Company during the
current calendar year will be used in a manner consistent with section 254(e); that is, for the provision,
maintenance, and upgrading of facilities and services for which the support is intended. The company
will continue to comply for the period of January 1, 2026, through December 31, 2026, to be eligible for
federal universal service fund support. CenturyLink QC anticipates it will relinquish its RDOF support
for Idaho in 2025.
6. This verification and affidavit is provided to the Idaho Public Utilities Commission to enable the IPUC to
certify to the FCC that federal universal service support received by the eligible carrier in the state will
be used in a manner consistent with Section 254(e) of the Telecommunications Act.

Corporation d/b/a CenturyLink QC (Southern Idaho), and
CenturyLink Communications, LLC
Dear Ms. Barrios-Sanchez:
This letter is to request that the Idaho Public Utilities Commission notify the
Universal Service Administrative Company ("USAC") and the Federal Communications
Commission ("FCC") that the above mentioned CenturyLink companies are eligible to
receive federal high-cost support in accordance with the above-referenced statute and
federal rule.
Enclosed please find the signed and notarized affidavit for the CenturyLink companies
certifying that they will only use their federal high-cost support received in Idaho for the
provision, maintenance, and upgrading of facilities and services for which such support is
intended. CenturyLink QC anticipates it will relinquish its RDOF support for Idaho in 2025.
This state certification for federal support will be an annual process to receive federal
support beginning January 1 of each year. The Idaho Public Utilities Commission must
file its annual certification on or before October 1 of the previous year.
CenturyLink respectfully requests that the Commission notify the FCC and USAC prior to
October 1, 2025 that the above-mentioned CenturyLink companies are eligible to receive
federal support for 2026.
