verification OQ records, verification equipment calibrations, verified proper rollback of completed butt
fusions, tracer wire, EFV 1200 installation, mechanical fittings (coupler)pressure test, squeeze off,
bedding, backfill and continuity test. Quizzed employees on AOCs of butt fusion and squeeze off
procedures. All observed procedures were completed in accordance with IGC Operations and Maintenance
Manual.
Observations Included: (19184 Smith Fork Way, Caldwell, ID) Correct trench depth, safety equipment,
verification OQ records, verification equipment calibrations, verified proper rollback of completed butt
fusions, tracer wire, EFV 1200 installation, mechanical fittings (couplers), pressure test, squeeze off
