Observations Included: (1269 and 1247 S Sabrina Ave, Kuna, ID) (Split Service) Correct trench depth,
safety equipment, verification of OQ records, verification of equipment calibrations, verified proper
rollback of completed butt fusions, mechanical fittings (couplers), tracer wire, pressure test, squeeze off,
bedding, backfill and continuity testing. All observed procedures were completed in accordance with IGC
Operations and Maintenance Manual.
Observations Included: (1285 and 1317 S Sabrina Ave, Kuna, ID) (Split Service) Correct trench depth,
safety equipment, verification OQ records, verification equipment calibrations, verified proper rollback of
completed butt fusions, observed fuser identify and cut out unacceptable fuse, mechanical fittings
(couplers), tracer wire, bedding and backfill.
Discrepancies:
At 1285 and 1317 S Sabrina Ave, Kuna, ID, IPUC inspectors and Dasco employees observed a live service
stub that was aboveground.
