Inspector Galli conducted a procedural audit of Avista Utilities. The Avista Utilities Gas Standards Manual
(GSM) and Gas Emergency and Service Handbook (GESH) were reviewed and compared to the PHMSA
Inspector Assistant(IA) question set.
June 3, 2025: (0.5 Standard) (Galli 0.5)
IPUC Inspector Galli of the Idaho Public Utilities Commission met with Avista's compliance and
engineering personnel via MS Teams to review any discrepancies. The PHMSA IA question set for the
records portion was used as a guide throughout the audit to ensure all areas were completed as they pertain
to required records.
Discrepancies:
1. 192.321(g)Uncased plastic pipe maybe temporarily installed above ground level under the
following conditions:
1. The operator must be able to demonstrate that the cumulative aboveground exposure of the pipe
does not exceed the manufacturer's recommended maximum period of exposure or 2 years,
whichever is less.
2. The pipe either is located where damage by external forces is unlikely or is otherwise protected
against such damage.
3. The pipe adequately resists exposure to ultraviolet light and high and low temperatures.
2. Avista will add a reference in the GESH to Avista's Cyber Security response.
3. GESH 13 states the correct requirements for reporting federally reportable incidents. However, the
procedure states they report to the DOT instead of the NRC. Avista will add NRC to procedures for
clarification.
A complete list of attendees can be found in the IA inspection. Completed IPUC Form 1 and the Post-
Inspection Written Preliminary Findings were sent to Randy Bareither, Pipeline Safety Engineer. NOPV
A202501 was written.
