Inspection Summary
May 19,2025: (1.0 Standard)
IPUC inspector Perkins traveled to Moscow, Idaho to inspect field installed operating equipment for the
Avista Moscow inspection unit.
May 20-21,2025: (1.5 Standard)
Met with Avista company representatives in the Pullman, Washington district office for a briefing,
reviewed welding records, past year work orders and began field audit of Moscow installed equipment.
Inspection was conducted on seven regulator stations, five single-service farm taps, and one odorizer to
assess relief operation, regulator lock-up, overall station condition—including pipe-to-soil wrap—and
cathodic protection. Additionally, four rectifiers, seven carrier-to-casings were checked for correct
cathodic protection (cp), and fifteen valves underwent inspection to ensure ease of operation.
Discrepancies:
-During an inspection at Regulation Station#343, the CP technician identified a short segment of steel pipe
that lacked cathodic protection. In response, the Pressure Control Technician returned to the station and
welded a jumper wire to ensure adequate CP protection
-On Rectifier 123758 at Palouse River Road, the CP technician was unable to obtain readings due to an
excessively high voltage detected on the rectifier housing. This issue was caused by the theft of the
grounding wire from the pole. A work order will be issued for the repair, and upon confirmation of the
grounding wire replacement, the rectifier will be re-evaluated next year

Returned to Boise and completed the inspection report along with necessary audit forms, including the
PHMSA IA question set with OQ Protocol 9, IPUC Form 1, and Post-Inspection Preliminary Written
Findings. Relevant inspection documents were also attached. Did site visit on Tamarack propane system,
investigated potential of Master Meter system.

Received workorder(4501077) for repair of ground wire on Rectifier 123758, confirmed completion/repair
on 12 Jun 2025.
