Inspection Summary
June 11, 2025: (2.0 Standard) (Galli 1.0) (Brooks 1.0)
Met with Avista company representatives in the Kellogg District Office for a briefing, reviewed welding
records, past year work orders and began the field audit of Kellogg installed equipment.
Ten regulator stations were inspected for relief operation, lock-up, general condition of the station
including pipe-to-soil wrap, and cathodic protection. Four rectifiers, fourteen carrier-to-casings, 4 bridge
crossings and twenty-one valves were also inspected.
Wrote inspection report, completed audit forms: PHMSA IA question set with OQ protocol 9,
IPUC Form 1, Post-Inspection Preliminary Written Findings and attached inspection documents.
