Inspection Summary
February 4, 2025: (2.0 Standard) (Galli 1.0) (Perkins 1.0)
Inspector Galli and Perkins traveled to the Rexburg LNG Plant to conduct a follow-up investigation of
prior compliance concerns and violations regarding:
1. Installation of mesh around PSV flanges on the vaporizer cart as recommended by CHI Engineering
Services on 23 April 2024.
-Vaporizer cart had all flanges wrapped in stainless steel mesh.
2. Perimeter security fence has significant gaps underneath in multiple locations as identified during CHI
Engineering Service report 23 April 2024 and IPUC field audit on 9 October 2024.
-No action had been taken as of the date of this inspection.
3. Structural condition assessment for the concrete cracking at the piers for the LNG tank and the
containment structure walls as identified by IPUC during 18 October 2023 field audit and Forsgren
Associates Inc. inspection report on 28 June 2024.
-No action had been taken as of the date of this inspection and saddles/supports were exhibiting signs of
crack propagation and additional foundation spalling.
February 5, 2025: (1.0 Standard) (Galli 0.5) (Perkins 0.5)
Inspectors Galli and Perkins traveled back to Boise.
