Upon arrival, I found that the project had been completed, except for applying the wax wrap to the 2-inch abandoned steel line and completing the backfilling. Upon notification of a line strike incident at 4028 Fairway Ave., where a contractor struck a %2" service line during the driveway installation, I promptly arrived on-site. Upon arrival, Avista service technician Brandon Hartill (OQ in file) was actively excavating the affected area and had applied a squeezer to halt gas flow. He was utilizing a four-gas detector to monitor vapors and had a fire extinguisher positioned nearby for safety. I inquired with the contractor regarding their locate ticket, and they directed me to contact the business owner. Upon speaking with the owner of the concrete company, he confirmed that no locate ticket had been requested, citing that Avista had flagged the area during a site visit two weeks prior. I informed him of the requirement to request a locate ticket before any excavation work. The ELM supervisor arrived on-site to verify the situation and confirmed that no locate ticket had been called in, ensuring that ELM was not at fault for the strike. The service technician proceeded with the necessary repairs, removing the damaged section and installing a new %Z"pipe, two feet in length, using Lyco fittings. A 100 PSI pressure test was conducted for ten minutes to ensure system integrity. Following the successful pressure test, the technician completed the supply side connection,released the squeezer, and performed a leak test on the repaired line. He then applied his operator number to the repair, marked off the squeeze point, buried the repaired section and conducted a relight at the location.
