Inspection Summary
April 21, 2025: (2.0 Standard) (Galli 1.0)(Brooks 1.0)
IPUC inspector Brooks and Galli traveled to Lewiston, Idaho to inspect field installed operating equipment
for the Avista Lewiston inspection unit.
April 22-23, 2025: (4.0 Standard) (Galli 2.0) (Brooks 2.0)
Met with Avista company representatives in the Clarkston, Washington district office for a briefing,
reviewed welding records, past year work orders and began field audit of Lewiston installed equipment.
Six regulator stations were inspected for relief operation, regulator lock-up, general condition of the station
including pipe-to-soil wrap, and cathodic protection. Five rectifiers, five carrier-to-casings and fifteen
valves were also inspected.
IPUC inspector Brooks and Galli traveled back to Boise, Idaho.
April 24, 2025: (0.5 Standard) (Galli 0.5)
Wrote inspection report, completed audit forms: PHMSA IA question set with OQ protocol 9,
IPUC Form 1, Post-Inspection Preliminary Written Findings and attached inspection documents.
Discrepancies:
Regulator(404) had a minor dithiazine buildup on the pilot stem, preventing full lockup and causing a
gradual rise in downstream pressure. The issue was resolved on-site by replacing the pilot stem.
