Request to Surrender Authorizations to Provide Intrastate
Telecommunications Services
Dear Ms.Jewell:
By this letter.Zayo Group.LLC (“Zayo”).as the successor in interest to Zayo
Bandwidth,LLC (“ZB”)that the Idaho Public Utilities Commission (“Commission”)
cancel the Registration filed by ZB with the Commission on Noember 20,2009,
effective immediately.In Idaho,ZB is a registered provider of non-regulated
telecommunications services pursuant to its notice filed in accordance with Rule 202,
IDAPA 3 1.42.01.
As a result of the Pm forma Merger,ZB no longer exists as a separate corporate entity
and Zayo has assumed ZB’s operations.Zayo.therefore,respectfully submits that the
withdrawal of ZB’s Registration and any effective tariffs is reasonable and necessary and
will not be disruptive to the present or future public interest,convenience and necessity.
An original and seven (7)copies of this notice are enclosed for filing.Please date-stamp
the extra copy and return it in the envelope provided.Should you have any questions
