Observations Included: (2354 Caliber Place, Blackfoot, ID) Correct trench depth, safety equipment,
verification OQ records, verification equipment calibrations, mechanical fittings (couplers), tracer wire,
pressure test, squeeze off, bedding, backfill and continuity tests. Quizzed employees on ADCs of squeeze
off procedures. All observed procedures were completed in accordance with IGC Operations and
Maintenance Manual.
Observations Included: (2355 & 2374 Caliber Place, Blackfoot, ID) Line locating, correct trench depth,
safety equipment, verification OQ records, verification equipment calibrations, inspected completed butt
fusions of fused T, mechanical fittings (couplers), tracer wire, pressure test, squeeze off, bedding, backfill
and continuity tests. Quizzed employees on AOCs of butt fusion procedures. This service line is a branch
service line feeding four services on one EFV 2600 and using a two-meter manifold connected to each
service line. All observed procedures were completed in accordance with IGC Operations and
Maintenance Manual.
Discrepancies: The crew found inadequate separation of the stub from an electrical conduit crossing. The
District Operations Coordinator was notified and advised the crew to wrap the service line with rock shield
to comply with OPS 307 Section 5.16.2.1.
