idaho Code liS' 61-1003 and 62-11 require that pubIc utility corporations annually report their -"\0J\ \\'5
gross operating revenues derived from business in Idaho during the preceding calendar year to the
Commission. We request that your calendar year 2006 gross intrastate revenues be provided to the
Commission on or before Aprill , 2007 You must report even if you had zero gross intrastate operating
revenues during the preceding calendar year.
The form provided below is for your convenience. Please complete and return it to the above
address as soon as possible. Please note the law requires information on the return be certified by an
officer, or agent, of your company. To meet the April I , 2007,. deadline you may send a facsimile of this
form to the Commission at [phone]. You may also complete the reporting form on the IPUC'
website at http://www:puc.idaho.gov/then copy it to your computer and finaily e-mail it to the e-mail
