Dear Ms.Jewell:
On April 1,2011,Qwest Communications International Inc.and its subsidiaries including
Qwest Corporation,Qwest Communications Company,LLC and Qwest LD Corporation
(“the Operating Entities”)completed their merger with CenturyLink,Inc.As a result of
the merger,the Operating Entities will operate with a CenturyLink doing business as
(“dba”)name effective August 8,2011.The entity names are as follows:
Qwest Corporation d/b/a CenturyLink QC
Qwest Communications Company,LLC d/b/a CenturyLink QCC
Qwest LD Corp.d/b/a CenturyLink LD
The legal entity names will remain unchanged.Revised title pages for the Operating
Entities’various Title 62 Tariffs/Catalogs and Price Lists will be filed with the
Commission to reflect the dba change.Additional Qwest entities that may operate in
Idaho,but not under the jurisdiction of the Commission,will also operate with a dba
name of CenturyLink.
This letter is being provided as a courtesy to the Commission.Please update the
Commission’s records to reflect the above-identified dba names as necessary and feel
free to contact me if you have any questions.
