RE: Election to make all Telephone Services in the I.P.U.C. No. 5 Tariff Subject to Idaho
Code Section 62 as amended

Dear Ms. Jewell:

Potlatch Telephone Company, Inc. d/b/a TDS Telecom hereby provides notice of its election to
make all of the Telephone Services in its I.P.U.C. No. 5 Tariff currently subject to Idaho Code
Section 61, subject to the provisions of Idaho code Section 62, as amended.

TDS Telecom is an incumbent rural carrier in Idaho, providing retail wireline services to both
residential and business customers. These include basic local exchange, custom calling and a
variety of other local services. TDS Telecom also provides switched and special access services.
In addition, TDS Telecom provides interconnection services as required by the
Telecommunications act of 1996 and FCC rules. These services are provided to customers in
Northern Idaho as shown in its exchange maps on file with the Commission.

TDS Telecom intends that all of the Telephone Services currently found in I.P.U.C No. 5 be
offered under title 62 and the access services in P.U.C. ID. No. 2 remain regulated under title 61.

Enclosed with this letter are new price lists that will replace the existing I.P.U.C. No. 5 tariffs. A
copy of the entire new price list including those items that have been price-listed previously has
been included for administrative ease.
