A regular full-time employee is one who works greater than 30 hours per week. All benefits start on the first of the month following your original date of employment. Medical and Rx • Company will pay 75% of the premium for employee plus eligible dependents. • Provider will be Regence for medical and Rx. Dental • Company will pay 75% of the premium for employee plus eligible dependents. • Provider will be Reliance for dental benefits. Vision • Company will pay 75% of the for vision. • The provider for vision benefits will be the Reliance. 401 K Benefit • Company match of 100% of employee's first 3% of elective contributions and 50% of next 2% of elective contributions (up to a maximum of 4% of eligible compensation). • The recordkeeper and administrator for the 401 K is 401 K Generation. Employee Stock Purchase Program (ESPP) • You will be eligible to participate in the company Employee Stock Purchase Program. You may enroll Dec 2024. Life Insurance/Accidental Death and Disability (AD&D) • Company provides Basic Life/AD&D insurance equal one times annual salary up to$100,000. • Employees may purchase voluntary supplement life insurance for themselves and buy life insurance for their spouse and children. Employee Assistance Program • Provided to all employees at no cost. Short-and Long-Term Disability Insurance • Short term disability benefits cover 60% of employee income up to $1,385 weekly for 24 weeks following a 14-day elimination period. This is a company paid benefit. • Long term disability benefits cover 60% of employee income up to $7,000 monthly. This is a company paid benefit. Accident Insurance • This is a voluntary insurance paid by the employee. • Provider will be TransAmerica for accident insurance. Flexible Spending Accounts (FSA) • Health and Dependent Care FSA's are available through Paychex. • Allows employees to set aside pre-tax dollars each year for eligible health care and dependent daycare expenses. Part-time employees, those who work less than 30 hours per week, will receive pro-rated holiday pay. • New Year's Day • Memorial Day • Independence Day • Labor Day • Thanksgiving Day • Day after Thanksgiving Day • Christmas Day • Veterans Day • Martin Luther King Day Floating Holidays • 4 Floating Holidays per year. Floating holidays are not carried over at the end of the year. Hire Date Days January 1 through March 31 4 April 1 through June 30 3 July 1 through September 20 2 October 1 through November I December 1 through December 31 0 Paid Time Off(PTO) [Combined Vacation and Sick] • Full Time Employees are eligible to accrue PTO as follows (part-time employees will have a prorated accrual of 1 hour for every 30 hour worked): Length of Continuous Service from Annual Accrual Anniversary Date Date of Hire, but less than 5 years 15 days At least 5 years but less than 10 years 20 days At least 10 years but less than 20 years 25 days Greater than 20 years 30 days Note: The above is a summary of the benefits for NW Natural Water of Idaho and any discrepancies between this and the actual benefits will be governed by the final plan documents.
