Observations Included: (Lake Hazel and Eagle Intersection, Meridian, ID) Line locating, correct trench
depth, safety equipment, verification OQ records, verification equipment calibrations, inspected butt fusion
process on 4"PE, electrofusion coupler of 4" PE, ran a 3/" aboveground bypass and then replaced with 2"
aboveground bypass due to low pressure issue in the adjacent neighborhood, tracer wire, squeeze off, partial
bedding and backfill. All observed procedures were completed in accordance with IGC Operations and
Maintenance Manual.
