REQUEST FOR PAYMENT OF ANNUAL REGULATORY FEES
This request is for $103.80 and must be paid on or before November 15 2005. The
amount is the total of the 2005 Annual Regulatory Assessment of$50., the unpaid balance of
$51.50 from the 2004 assessment, and the related interest on the all unpaid balances of $2.30.
The commission is required to charge interest of 6% per year on amounts past due.
Because companies may elect to pay their Annual Regulatory Assessment in two parts, the first
half was due on May 15 , 2005 and the second half of their assessment is due on November15th.
The unpaid first half payment is subject to interest as well as the outstanding balance from last
year. As of this date, our records show that your company has not paid either half of your
assessment or the amounts past due.
If your company no longer conducts business in Idaho please have an officer of your
company complete the certification at the bottom ofthis request. Should other circumstances
have occurred that prevent payment ofthe Annual Regulatory Fee please contact me.
Public Utilities Law provides for civil penalties up to $2 000 per day for non-compliance
with Idaho Law, commission orders and/or directives. To avoid further action please respond
promptly to this request for payment.
