Idaho Code Sections 61-1003 and 62-611 requires that utility and railroad companies annually report their
gross operating revenues derived from business in Idaho during the preceding calendar year to the Idaho
Public Utilities Commission. We request that your calendar year 2000 gross intrastate revenues be provided
to the Commission by March 9, 2001 but in no case later than March 31 , 2001. You must report even if you
had zero gross intrastate operating revenues during the calendar year.
The form below is provided for your convenience. Please complete and return it as soon as possible. Please
note the law requires information on the return be verified by an officer, or agent, of your company. To meet
the March 31 , 2001 , deadline facsimile transmissions are acceptable. However, the signed originals must be
