Observations Included: (11069 Canvas St) Correct trench depth, safety equipment, verification OQ records,
verification equipment calibrations,purging,pressure testing, tracer wire install, fusing, mechanical
fittings (couplers), squeeze off, EFV 1200 installation,bedding and backfilling. Quizzed employee on
squeeze time procedures.
Observations Included: (17630 Mason Ridge Way) Line Locating, correct trench depth, safety equipment,
verification OQ records, verification equipment calibrations, locates markings verified, purging,
pressure testing, tracer wire install, squeeze off, fusing, mechanical fittings (couplers), EFV 1200
installation, bedding and backfilling.
Discrepancies: The pressure test conducted at 17630 Mason Ridge Way, Caldwell, ID, was attempted three
times due to a leak on the pressure test equipment(tree).During the final attempt,the pressure test experienced
a loss of approximately 2 psi within the 15-minute testing period. The IPUC inspector observed the small leak
from one of the threaded connections on the tree and advised the OQ'd contractor accordingly. Despite the
leak,the test was deemed successful by the OQ'd contractor,and the system was subsequently put into service.
