Dear Mrs. Jewell
The purpose of this filing is to request the correction of PUC records to reflect the actual existing
Company, Frontier Communications of America, Inc. and to modify the existing title 62, Price
List on file with the Commission , to reflect the correct Company name.
The Commission s records contain two separate entities that are in fact the same Company;
Frontier Communications of America, with Commission Identification letters CTZ, and Frontier
Communications of America, Inc, with PUC identification letters FCA. The Correct name of the
Company is Frontier Communications of America, Inc. This Company is a subsidiary of Frontier
Communications corp. and is an affiliated Company of Citizens Telecommunications of Idaho,
dba, Frontier Communications of Idaho. The existence of two different names for the same
Company has become very confusing for both the Company and Idaho PUC staff. We therefore
request that the current name for the company with the identification letters CTZ be changed to
Frontier Communications of America, Inc. We also request that all documents and filings under
the company identification letters FCA be moved to Company identification letters CTZ.
The current Title 62 Competitive Local Exchange and Network Services Price List Frontier
Communications of America is shown under the name Citizens Telecom. On December 9, 2003
the Commission issued Order No. 29394 in case No. CTZ-03-01. This order approved the name
change of Citizens Telecommunications Company dba Citizens Telecom to Frontier
Communications of America. However, the current Price List still reflects the name Citizens
Telecom. The attached Price List changes the name on the Price List to the correct Company
name, Frontier Communications of America, Inc.
Frontier intends that this filing will become effective on October 19 , 2009.
