Tami,for 1XC companies like Entrix all we need is a letter in the form of an email addressed to [e-mail]
asking us to cancel their price lists or a completed Annual Report Form (available on our website)wherein they can
check a box at the bottom of the form to indicate they want to cancel.Angie said that Entrix completed the form in
January 2013 and typed “NO”in the box to indicate they did not want to cancel their price lists.If they send an email I
always acknowledge receipt of the email and forward the information to Angie.It is not likely that we missed his email
or letter requesting cancelation.However,based on Mr.Billek’s email below,we can go ahead and cancel his tariff
now.

Hi Jean.
I need some assistance regarding the string of emails below.
Mr.Billek provided some information below related to a couple companies and submitting the 2014 Idaho Intrastate
Gross Operating Revenue Reports.I am specifically trying to figure out what information to provide him based on the
highlighted sentence below.
What I think happened was Mr.Billek contacted the Secretary of State’s Office and withdrew their company registration
and thought that was all he needed to do.Before,I responded back to him I wanted to make sure the PUC did not
receive any thing and we missed de-activating the company in UIS and QuickBooks.I forwarded the information to Anga
for assistance and it looks like we did not receive anything.
My question is what do we need from Mr.Billek to cancel/remove Entrix Telecom Inc from the assessment list?
Thanks Jean.Sorry for the long explanation,I just want to make sure that I provide the correct information back to our
customer so we can address the issue and de-activate them in our systems.

I need some assistance relating to Mr.Billek’s comment about Entrix Telecom Inc.(highlighted below).I am thinking he
contacted the Secretary of State but want to confirm that we have not received any notice of remove the company from
the list.Do you have any information in any of the files to support is statement?

Ms.Humiston:
Two things:(1)there is a bit of a problem that I would like to resolve (more on that in a moment);and (2)1 have
attached IDT’s report.
I was at first confused by your request as I recalled submitting a report.But when I looked back at the report submitted
(it was on the pre-printed form sent to me by the Commission),I noticed that the addressee was actually Entrix
Telecom.Entrix was an affiliate of IDT and I handled its reporting so it’s not out of the ordinary that I would receive a
form for them.But what is out of the ordinary is that I notified the Commission in the end of 2013 that Entrix ceased
operations effective 12/31/13 so I should not have filled out (nor do I expect you would want me to fill out)any
information for 2014.Also,please note that the $listed on the Entrix form is incorrect —the actual amount is $0 (I put
IDT’s $on the Entrix form in error.)
Please let me know if I need to do something to ensure that Entrix is no longer on the Commission’s files as an entity
providing telecom service in the state.
