Inspectors Perkins and Galli reviewed the Rexburg LNG plant Records, Emergency Manual,Fire Manual,
Operations Manual,Preventative Maintenance Guide,Plant Maintenance Training Manual,
Equipment Operations Manual and Security Training Manuals for compliance with 49 CFR part 193,
NFPA 59A and NFPA 10.
October 7, 2024: (2.0 Standard Travel/Standard)(Galli 1.0)(Perkins 1.0)
Inspectors Perkins and Galli traveled to IGCs Rexburg LNG Plant and conducted field observations.
October 8,2024: (1.5 Standard Travel/Standard)(Galli 1.0)(Perkins 0.5)
Inspectors Perkins and Galli traveled back to Boise and started Form 1.

Inspectors Perkins,Brooks and Galli of the Idaho Public Utilities Commission met virtually with Intermountain
Gas Company personnel Colby Lundstrom, Darren Tinnerstet,Wendy McDonough,Russ Nishikawa,Daniel
Osterhout and Chad Darrington to conduct an audit of procedures and records for the LNG plant operations for
calendar year 2023. The PI MSA Inspector Assistant (IA)Program was used as a guide to conduct the
inspection. An IA Post Inspection Preliminary Findings(exit interview)was performed following the inspection
and sent to Colby Lundstrom.

Field Observation Discrepancies
1. The first aid equipment and eye wash station were found to be expired for many years.
2.Additionally, the perimeter fence line was found to have approximately a foot gap between the ground and the
bottom of the fenceline in some areas.
Procedural and Records Discrepancies
1. OPS 9 Section 8.6.2 needs to reference LNG to match code 191.23(a)(3)
2. First Aid material was observed expired. Procedure needs to include an inspection interval.
3. Procedure 4565.4 Rexburg LNG Transfer Operations needs to reference code section 193.2513 (b)(4)
4. No reference to annual auxiliary power source capacity testing in procedure 4558.3. Prior LOC 1202310.
5. No atmospheric corrosion procedure for the Rexburg LNG Plant.
6. IGC self reported that the emergency manual has not been reviewed since 2014. NOPV 1202415 issued.
7.Daily and Monthy plant checklist's were found with multiple inconsistencies between VAP techs.
8. In 2023,there was no cooldown of components for vaporization checklist in accordance with 193.2505.
9. July 2023 monthly checklist does not include monthly generator testing. Shows date of October 5, 2023.
10.Initial and Recurrent training manuals state the Supervisor of Engineering Services shall be trained on
Operations, Maintenance and Fire Protection. No records of initial training found for Supervisor.
Per IGC, Supervisor of Engineering Services will be changed to Lead VAP Tech in the training manual.
11.No initial training records for multiple VAP Techs due to unverifiable test records.
12.No continuing education training records will be available until January 2025.
13. Section 11 of initial training test not completed or signed off for any VAP tech.
14. IGC was unable to provide a full Fire Evaluation from CHI.
