Address: 16342/16330/16318 Grade Ave Caldwell, Idaho
Observations Included: Line locating, correct trench depth, safety equipment, verification OQ records,
verification equipment calibrations, tracer wire install, mechanical fittings (couplers), fusing, EFV 1200
installation, successful pressure test at 96 psi for 15 minutes, cleaning purge, bedding, backfill and
continuity test. On 16342 Grade Ave, fuser did not successfully complete pressure test to include all
fittings, did not shade before pressure test and attempted to stab house service line onto service stub
before pressure testing. All items were corrected by(IPUC, IGC and Das-Co leadership)prior to service
line being put into service. Quizzed employee on line locating procedures, and squeeze times.
