Office Manager REQUEST NO. 1: Please provide a nine-year water monitoring report that describes the frequency of each water test and its respected testing cost. • See Jump Drive Request#1 • A PDF of Magic Valley Labs (who did our water testing until 12/2023) QuickBooks report of all transactions our company has had with them over the last 10 years. • A water Monitoring QuickBooks Report of our transactions since I have taken over the books in 01/2018) • I also included a DEQ Public Drinking Water System Monitoring Schedule Report that shows what testing we will need in (FUTURE) reports up to 12/2032. REQUEST NO. 2: Please provide a copy of the bank statements for all accounts used by the Company for 2023 and 2024 to date. • See Jump Drive Request#2 • I have provided two Years of Bank Statements. We only have one Bank we use for Aspen Creek Water System. REQUEST NO. 3: Please provide the expected cost for processing this rate case application. • See Jump Drive Request#3 REQUEST NO. 4: Please explain if there are different connection sizes offered to serve customers. If so, please provide a current count of connections for each connection size. • 1" CTS is the only connection offered. We only provide to Residential Customers no Commercial Customers are on this system. REQUEST NO. 5: If the service connections to customers are metered, please explain the Company's policies and procedures regarding the reading of customer meters. • Meters are placed at all customers' homes. We currently do not read the meters and have charged a flat rate per customer since we took over the books in 2018. REQUEST NO. 6: The Company's Application indicates that it charges customers a fixed un-metered monthly rate. Please explain if the Company charges all customers a fixed un- metered rate. • We currently do not read the meters and have charged an annual flat rate per customer since we took over the books in 2018 REQUEST NO. 7: Please provide the number of customers by connection size billed monthly for each month from January 2022 through September 2024. Please explain any known reasons for monthly variations in the number of customers billed. • We bill annually. The map should include the location and elevation of major equipment including but not limited to wells, distribution piping, storage tanks, booster pumps, shut-off valves, meters, fire hydrants, water treatment equipment, etc. Please provide in electronic format(.pdf)which can be scaled and enlarged to identify all aspects of the system. • See Jump Drive Request#8 REQUEST NO. 9: Please provide a thorough and complete description of the water system including the following details regarding the system as applicable: a. The depth, width, length, capacity, construction materials of each well (including backup wells, if any), and length of time in service. • #1 Loveland Well 12" 350' deep Steel Case Drilled 1978 Culinary 1998 • #2 Gentle Ben Well 10" 350' deep Steel case Drilled 2007 not in service. b. The make, model, type, and rating of each well pump motor, and length of time in service. • #1 Loveland Pump 40 HP Hitachi • #2 Gentle Ben 40 HP not in service c. The make, model, capacity, and limit switch settings of each pressurized storage tank, and length of time in service. • Not Applicable d. The make, model, capacity, and limit switch settings of each unpressurized storage tank/reservoir, and length of time in service. • Partially above ground concrete storage reservoir was built in 1998 so has been in service for 26 years. It has a storage capacity of 115,000 gallons at the 16 ft level. e. The make, model, and rating of each booster pump and motor, and length of time in service. • No booster pumps. f. The make, model, and capacity of all water treatment equipment, and length of time in service. • Not Applicable g. The year, make, model, and capacity for backup generation supporting the well pumps, and length of time in service. Indicate if the generation has an Automatic Transfer Switch (ATS) or if it must be manually started. • Manual switch was purchased and on back ordered finally got delivered this year but needs to still be installed by electrician. h. The distribution piping line size and materials specified in the installation, and length of time in service. • 6" & 8" C900 PCU REQUEST NO. 10: For each well used by the water system,please provide the following: a. The Idaho Department of Water Resources Well Tag Number(D-Tag Number) that is currently on-file with IDWR; and • ID#A6015882 /Loveland Well • ID#D0043948/Gentle Ben Well b. Location(Township, Range, Section, and Tract). • Loveland Well/16S 43E 15 SW SE Bear Lake County 0 Gentle Ben Well/ 16S 43E 22 NE NW Bear Lake County REQUEST NO. 11: Please provide a copy of the Company's most up-to-date Facility Plan of its water system. • We currently don't have a facility plan on file. REQUEST NO. 12: Please provide detailed information related to the capacity and sizing of the system and the ability of the system to meet historical and future system peak demands. Information should include not only capacity or constraints of or within the system, but also consumption and consumption at peak periods. Please provide necessary workpapers in spreadsheet(such as, Microsoft Excel) format with formulas intact. REQUEST NO. 13: Does the Company expect customer growth? If so,please provide workpapers and documentation including but not limited to the potential number of customers and type of customers by year for the next five years. NO further development is planned for the water system. The only growth will be if vacant lot owners decide to build homes. REQUEST NO. 14: Please provide a copy of the Company's latest Idaho Division of Environmental Quality (IDEA) Sanitary Survey. See Jump Drive Request#14 REQUEST NO. 15: Please provide the monthly power bills indicating the amount of electricity (kWh) used and corresponding electricity cost(in US Dollars) for each well and booster pump from January 2019 to December 2023. Please provide your responses in spreadsheet(such as, Microsoft Excel) format with formulas intact. See Jump Drive Request#15 REQUEST NO. 16: Please provide the monthly metered amount of water(gallons) pumped from each well from January 2019 to December 2023. Please provide your responses in spreadsheet (such as, Microsoft Excel) format with formulas intact. • See Jump Drive Folder Request#16 (This is the info we have after we fully took over the water system,we started monitoring the Loveland Well for our own knowledge and to get firm numbers to give the accountant for the Annual Report.) REQUEST NO. 17: Please provide the monthly metered total customer water usage data (gallons)by customer types from January 2019 to December 2023. Please provide your responses in spreadsheet (such as, Microsoft Excel) format with formulas intact. • We do not have this information we have just charged flat rates and never monitored individual customers. REQUEST NO. 18: Please provide the total monthly amount of chlorine used(in pounds), and total expense for chlorine purchased (including Sales Tax) for each well from. Also please specify whether a solid or liquid form of chlorine was used for each well. Please provide your responses in spreadsheet(such as, Microsoft Excel) format with formulas intact. • We do not chlorinate unless there has been construction or fixes have taken place. We use 2 lids full per month in those cases. REQUEST NO. 19: Please provide the average monthly pressure data in the main distribution line from January 2019 to December 2023. Please provide your responses in spreadsheet(such as, Microsoft Excel) format with formulas intact • Unknown. Our water system is a gravity fed. Water is pumped up to the holding tank from there dispersed thought the system. REQUEST NO. 20: Please provide the definition, scope, detailed and itemized list of components or materials used, labor hours, and breakdown of individual costs for each item needed for each of the following items: • See Jump Drive Request#20 a. Install meter and turn on water only ($500); o Meter Parts $235.30 (see attached Peterson Plumbing Proposal) / Labor$264.70 b. Pit setter and meter only ($2,655); o See attached invoice From Dirthead Services - Hot Tap C. Pit setter,meter, and tap main line ($3,005); o See attached Invoice from dirthead Services 2024-039 o Pit setter, meter,tap main line+ excavation or horizontal boring across road($7,505). See attached invoice+ estimate from Double Diamond REQUEST NO. 21: Please provide justification and necessary documentation(including but not limited to prior invoices, bidder estimations, etc.) on how the Company calculated or estimated the new connection charges as outlined in Application Page 1. • See Jump Drive Folder Request#21 REQUEST NO. 22: In reference to the previous two production requests (PRs 20 and 21), please explain which of the above-mentioned body of works the Company is planning to perform using in-house labor, and which ones will be completed by third- party vendors/contractors. If the Company uses a third-party vendor/contractor to complete either of these categories of installations,please explain the process of selecting them (such as RFP/RFQ, etc.)to ensure it will be performed at the least-cost to customers. • Horizontal boring has been done by a third-party Double Diamond in Burley. • Plumbing Supplies have been purchased through Peterson Plumbing in the past,but we have reached out to Core and Main in Logan because they have had things available. • Dirthead Services would do pit setter meter and hot taping water main. They are insured and have the equipment if something goes wrong. We have worked with Dirthead Services in the past because they were the developer of part of Aspen Creek Meadows and installed most of the infrastructure of the water system. • All outsourced Jobs will be bid out to keep all contractors priced in reason. • Installing meter and turning on water would be done inhouse by Aspen Creek Water Co Operator after customer fees are paid. REQUEST NO.23: In"Exhibit 1, Schedule A" of the Application, the Company listed the following plant in service item as known changes: a. Structure Improvements ($68,655) for PRV and Well House. For the above-mentioned items, please provide the following documentation. If any of the information requested below cannot be provided or is not available, please explain why; also explain how the Company can ensure the project was completed at least cost. PRV was done at cost labor was for assembly of PRV only. i. A justification of need for the project; • High water pressure due to elevation from tank made it so that PRV was needed for the Stockyard Expansion to drop pressure. ii. Copy of requests for proposals ("UP") or requests for quotes ("RFQ"); iii. Project construction contracts, invoices; 0 See Jump Drive Request#23 PRV Documentation iii. In-service date. • 09/30/2024 Stockyard Expansion complete Well House we have had 2 Bids for the well house project. Dirthead Services believes they can build Well House at a lesser price. i. A justification of need for the project; • The state of Idaho Requires a second source for the water system. The well house is essential to tie in the second source to the system. ii Copy of requests for proposals ("RFP") or requests for quotes ("RFQ"); iii. Project construction contracts, invoices; • See Jump Drive Request#23 Pump House Bids iv Detailed breakdown of the actual costs; and • See Jump Drive Request#23 Pump House Bids v In-service date. • We are hoping for a service date of 12/31/2024 REQUEST NO. 24: In"Exhibit 1, Schedule A" of the Application,the Company listed the following plant in service item as known changes: a. Supply Mains ($42,000) for plumbing supplies for connecting wells. For the above-mentioned items,please provide the following documentation. If any of the information requested below cannot be provided or is not available, please explain why; also explain how the Company can ensure the project was completed at least cost. i. A justification of need for the project. • The state of Idaho Requires a second source for the water system. Plumbing and electrical materials are needed to put the well in service. ii. Copy of RFPs or RFQs; • We are working on getting bids for the plumbing and electrical install. iii. Project construction contracts, invoices. 0 We are working on getting bids for the plumbing and electrical. iv Detailed breakdown of the actual costs; and We are working on getting bids for both plumbing and electrical. v. In-service date. • This project will be done after Well House is completed. REQUEST NO. 25: Please provide a list of additional plant in service expected to be added through year-end 2024 and, if any,please provide the expected cost, including labor. • Other than The PRV and Well House there are no other projects to be completed by the end of 2024 REQUEST NO. 26: Please provide a spreadsheet detailing Company's plant in service, or assets, separately identified by type, place into service, amount, and depreciable live, as of June 30, 2024. Please include in your response the following: • See Jump Drive Request#26 This is the best I have for now! Our taxes and depreciation are done once a year we are currently looking for a new accountant! a. original price of the asset; b. net book value of the asset; c. date asset placed in service; d. current annual depreciation expense; e. Accumulated depreciation; and f. any receipts or backup documentation for all plant in service purchases since 2023. REQUEST No. 27: does the company intend on using meters in its distribution system to bill customers for actual use? If not,please explain why meters will not be used for billing purposes? • Yes,we do! When we took over the water system the previous operators were installing meters and we continued to do so,with the intention of metering customers at some point in the future. REQUEST NO.28: If the company does not intend on using meters for billing purposes, please explain why the company intend on charging new connections for installation of meters and pit setters. 0 Not Applicable Please Refer to answer to Request No 27
